| Eligibility | Accepts referrals for frail elderly individuals, primarily aged 75 and older who live at home or in a retirement residence, have multiple medical problems, have experienced a recent decline in their independence or a recent fall, and have limited social supports | |||

| Description | Specialized geriatric services to the frail elderly residents in the Township of Brock and north Durham * comprehensive geriatric assessment in a clinic environment with a specialized nurse practitioner * access to a geriatrician via teleconferencing in a clinic setting * home assessments by an interdisciplinary team (if necessary), follow-up and support * links with the client's current primary care provider to other specialized geriatric services | |||
