DescriptionHOME SUPPORT PROGRAM 
Services that support adults and their caregivers with needs related to aging, physical and/or mental health * assists clients to remain in their own homes and communities, and to function as independently as possible  
 
Home Support Services  
 
Transportation  
Drives for clients to medical appointments, shopping and personal business * for Clarington clients, a specially designed wheelchair accessible van is available 
 
Home Help 
Light housekeeping assistance and help with errands  
 
Home Maintenance 
Occasional minor repairs, yard work, grass cutting, snow shovelling or one-time clean-up  
 
Meals on Wheels™  
Nutritionally balanced meals to people who are unable to prepare meals or easily shop for groceries * hot or frozen meals are delivered by volunteers * special diets are available  
 
Luncheon Out  
An opportunity for adults with physical disabilities and the frail elderly to enjoy a nutritious meal, information and/or entertainment in a local, centrally-located setting * luncheons are held at preset times * transportation is available if required 
 
Friendly Visiting  
A volunteer to visit on a regular basis with lonely or isolated clients * activities may include playing cards, taking walks, doing crafts or just enjoying pleasant conversation  
 
Telephone Reassurance  
Daily telephone contact with isolated clients offers reassurance and help when needed * service may be provided on an ongoing basis or when family and friends are away  
 
Foot Care  
Community Clinics provide foot care by registered practical nurses * regular appointments are booked for clients at each site
