Mental Health and Primary Care Centre promotes and enhances the mental, emotional, and physical well-being of community * education, collaboration, advocacy, and a broad range of direct services

Community Connection Services Centralized intake responds to all inquiries about programs and services * information and referral * short-term Case Management, Criminal Justice Case Management * links to external programs

Nurse Practitioner-Led Clinic and in-house pharmacy Health care to individuals and their families who do not have a family doctor * cardiovascular health * medication management and Clozapine Clinic, Diabetes Management * nutrition and health teaching, vaccinations * Healthy Living Program, Chronic Pain Management Program * links to Telemedicine for education and consultation

Housing and Case Management Services Supportive services for those in group living or independent living situations * community-based suitable, affordable, safe housing to people with mental health difficulties

Assertive Community Treatment (ACT) Team Interdisciplinary, mobile, team-based intensive services * partnership with Lakeridge Health Oshawa * transitional Rehabilitation Housing Project in partnership with Ontario Shores, intensive case management, housing supports for individuals with an NCR status

Community Treatment Order Service (CTO) Intensive case management to individuals with treatment orders * partnership service with Lakeridge Health Oshawa, Ontario Shores, Ontario Shores ACTT, Rouge Valley ACTT

Community Wellness Services Opportunities to participate in self-directed activities that assist individuals to connect in their community * Life Skills Program, Older Adults Wellness Program, Peer Supports and Initiatives, Rehabilitative Group Living, Collaborative Living * Small Business Initiatives (SBI's), Youth Services - Public Trustee, Youth Activity and Support Program (YASP funded through the Royal Bank of Canada's RBC After School Grants Program) after school activity and support program for youth attending high school and living independently

Health Promotion, Public Relations Events, workshops, displays, information, educational workshops and training in Durham Region * training includes: Applied Suicide Intervention Skills Training (ASIST), Suicide SafeTALK, Mental Health First Aid (MHFA), Mental Health First Aid for Adults who Interact with Youth, Living Life To the Full, Strengthening Children's Mental Health and Mental Health Works

Corporate Services Integral supports such as Human Resources, Finance, Facility and Property Maintenance, Risk Management, IT, office services to CMHA Durham * fee for service including property management, bookkeeping and accounting services
