| Description | Durham Health Connection Durham Health Connection Line (DHCL) is a free confidential telephone help line available to people living, working and visiting Durham Region * public health nurses access information and resources to protect and promote health * DHCL is the front line connection to the Public Health Nursing and Nutrition Division programs and services * healthy living, parenting issues, breastfeeding * immunization, injury prevention * sexual health and nutrition Reproductive and Child Health Program - RCH Parenting programs enable families to access resources, support networks and develop healthy environments * supports healthy pregnancies, promote optimal health and development in children * provides breastfeeding support and promotes parenting skills * programs ensure children achieve optimal potential, delivered by Public Health Nurses, Public Health Nutritionists and Family Visitors Health Promotion and Disease Prevention Program - CDP Public Health Nurses and Public Health Nutritionists deliver provincial mandatory health programs and services to address chronic disease prevention, early detection of cancer and portions of the Sexual Health Standards * implement the Smoke Free Ontario Strategy and the Public Health School Asthma Pilot Project Prevention of Injury and Substance Misuse Program - PISM Programs for Durham Region residents, healthcare professionals, parents and guardians, students, employers, key stake holders and seniors in order to prevent and reduce injuries, disability, morbidity, and mortality caused by motorized and non-motorized vehicles, alcohol and other substances and falls Infectious Diseases Prevention and Control - IDPC Prevention, reduction or elimination of infectious diseases including vaccine preventable diseases, tuberculosis, sexually transmitted infections and through the promotion of healthy sexuality * Public Health Nurses and Community Health Nurses deliver clinical services, also community surveillance and investigations and promotional activities | |||
