Description (Service) | Durham Health Connection Line Durham Health Connection Line (DHCL) a free confidential telephone help line available to people living, working and visiting Durham Region * nurses provide health information and access to resources to protect and promote health * DHCL is the front line connection to the Population Health Division programs and services * healthy living, chronic disease and injury prevention * healthy pregnancy and parenting support, breastfeeding * immunization, * sexual health and *nutrition Healthy Families Program Enables families to access resources, support networks and develop healthy family environments * supports healthy pregnancies, promotes optimal health and development in children * provides breastfeeding support and promotes parenting skills * programs ensure children achieve optimal potential, delivered by Public Health Nurses, Public Health Nutritionists and Family Visitors Infant and Child Development Early intervention and prevention services * available to families with children birth to 6 years (limited service from 3-6 years) * promotes optimal development and school readiness * child and family support and education Healthy Living Program Provides health programs and services to address the prevention of chronic diseases, injuries and substance use in order to improve well-being * Collaborate with school boards and schools to achieve optimal health of school-aged children and youth Infectious Diseases Prevention and Control Program Prevention and control of infectious diseases through case and contact management * infectious disease surveillance and outbreak investigations * sexual health clinics * sexually transmitted infections testing and treatment Immunization Program Complete surveillance of students’ immunization records in accordance with legislation * Provide access to immunizations in schools and community clinics * Investigate reports of adverse events following immunization * Distribute publicly-funded vaccines to health care providers * Educate and support health care providers related to vaccine management and storage and handling practices |
