Description (Service) Program for children who have a short term or long term illness, a physical disability or require assistance due to an injury or recent hospital stay * services may be provided to students in publicly and privately funded schools, junior kindergarten to high school * service provided in the school or home of client

Eligibility Ages: 18 year(s) and under 
Students must have a valid Ontario Health Insurance Plan card * needs of the student cannot be met on an out-patient basis * student requires at least one of the professional services available through the Central East Local Health Integration Network
