| Description | HOME SUPPORT PROGRAM Services that support adults and their caregivers with needs related to aging, physical disability or special needs * assists clients to remain in their own homes and communities, and to function as independently as possible Transportation Drives for clients to medical appointments, shopping and personal business Home Help Light housekeeping assistance and help with errands Home Maintenance Occasional minor repairs, yard work, grass cutting, snow shoveling or one-time clean-up Meals on Wheels™ Delicious, nutritionally balanced meals to people who are unable to prepare meals or easily shop for groceries * hot or frozen meals are delivered by volunteers * special diets are available Luncheon Out Opportunity for adults with physical disabilities and the frail elderly to enjoy lunch and an outing * luncheons are held at preset times * transportation can be arranged Friendly Visiting Volunteer visits on a regular basis with lonely or isolated clients * activities may include playing cards, taking walks, doing crafts or just enjoying pleasant conversation Telephone Reassurance Daily telephone contact with isolated clients * reassures them that help is available when needed * ongoing basis or when family and friends are away Foot Care For those who because of medical conditions cannot manage their own foot care * foot care is provided by a nurse trained in basic and/or advanced foot care in a clinic setting * includes trimming toe nails, massaging and monitoring the condition of the feet * education on caring for foot problems | |||
