DescriptionSingle service, inter-professional rehabilitation service * specialized assessment in an outpatient setting for patients who have experienced a stroke, brain injury or have complicated orthopedic conditions or neurological challenges * patients gain skills, information, receive support to achieve ordinary living  
 
Team includes physiotherapists, occupational therapists, speech language pathologists, social work and rehabilitation assistants * other disciplines are available by consultation
